I worked off-shore in the Gulf of Mexico. It was a heavy construction/industrial environment, but the majority of our accidents were basically the same dumb things in this video; i.e. people not looking where they were going, not paying attention, slipping or tripping over small things, not using hand-rails, using the wrong tools/equipment for the job, throwing a still-lit cig in the trash-can, and trying to lift too much weight alone or using wrong lifting techniques.
